How can you help my loved one with Dementia and/or Alzheimer’s?
I am a Certified Dementia Practitioner as well as developed and oversaw the operations of a Memory Care Cottage. We can discuss your specific situation and my services will help guide you on the day-to-day as well as care options that are available.
Can you do a virtual assessment as well as on-site?
Yes. I can work with families anywhere. We can set up video meetings that allows us to see and conduct the consultations.
Are you Clinical?
No. I will not provide any clinical assessments. If there is a need for a healthcare provider, I will work with Caregiver and Family to ensure Care Recipient seeks medical attention. However, many times, it is the navigation and care coordination that is difficult, and my services can allow you to focus on being a family member while I oversee the care coordination and navigation of the healthcare system.
Can you take a care recipient through the continuum of care?
Yes. Whether the care recipient is living at Home, Assisted Living or in the hospital, we can help coordinate care from any place and coordinate services with various healthcare providers and post-acute services to the end of life.
Is there a caregiver support group available?
Yes. It meets monthly on the first Tuesday of each month at noon. Contact us for more details.
How much do you charge?
When working with an individual or family, I offer 3 tiers of services that varies based upon length of engagement. Please send us a message to set up a 30-minute introductory call by scheduling an appointment.
